How to register with our Doctors

Anyone living within our practice boundary is welcome to apply to register with us. Prospective patients are asked to bring their medical card to the surgery (don't worry if you can't find it, we can give you a form to fill in). Should you have a preference, you may indicate which doctor you wish to register with if they are taking on new patients.

All new patients joining the practice will be asked to attend for a simple medical examination with one of the nurses at which their general health will be assessed. This enables us to obtain some basic medical history before your medical notes arrive from your previous doctor.

Change of Personal Details

Please inform reception if you change your name, status, address or telephone number, especially mobile telephone numbers. You will be asked to complete a form to ensure that your records contain up-to-date information or you can complete our on-line form by clicking here.
